Work Permits

Application for student work permits can be made in the Business Office at Appleton West High School.

The following must be presented at the time of application:

  1. Certified copy of birth certificate, baptismal certificate, passport, or State ID.
  2. Letter from the employer stating the hours to be worked and the duties to be performed. Please include the employer's address and phone number.
  3. Letter from parent/guardian providing permission for the student to work.
  4. Social Security Card (Social Security number cannot be given from memory or from any document other than a verification form from the Social Security Office if the original card has been lost or misplaced.)
  5. There will be a $10.00 fee. This fee is reimbursed to the student by the employer.

In order to do the necessary paperwork, work permits can be done between 8:00 a.m. and 2:00 p.m., Monday through Friday on school days. Office hours on non-school days vary, so please contact the school before you come in to make sure that a permit officer will be available.
